Welcome to the Champlain Local Health Integration Network

The Ontario government created 14 LHINs to plan, integrate and fund local health services in each region.

To learn more about the Champlain LHIN’s mandate, successes and strategies, watch our new introductory video.

The Champlain LHIN is issuing a call for proposals from approved health service providers to provide exercise and falls prevention classes in our region. For more information click here.

Health Links

A Health Link is a new way of organizing health care services for people with the highest, most complex needs. Health Links involve many health providers, including primary, home, community care, specialists and hospitals, in coordinating care plans for these individuals.
