Close to 1,000 Champlain residents without a family health care provider have now found one thanks to the Health Care Connect program. Launched in February 2009 by the Ministry of Health and Long-Term Care (MOHLTC), and being administered by CCACs across Ontario, Health Care Connect links prospective clients to family physicians and nurse practitioners.

The framework is a living document as ongoing strategies and approaches are informed by experience and evaluation.
