Facilitated Integration Decision Concering Convalescent Care Program

This Facilitated Integration Decision resolving the issue of convalescent care beds involves three health service providers: Specialty Care Granite Ridge, Perley and Rideau Veterans’ Health Centre and Garden Terrace (OMNI Healthcare Limited Partnerships).

The purpose of this proposed integration (transfer) is to:

• Enable a smooth transition of the convalescent care program within the Champlain LHIN;
• Maintain high quality of care and equitable access for clients of the convalescent care Program across Central Champlain;
• Maintain access to long stay capacity; and
• Improve the utilization of the short stay capacity.
