Integrated Health Service Plan

This plan explains how the Champlain Local Health Integration Network will begin transforming the region’s approach to health, to create a system that is more effective, compassionate, and responsive to the health needs of our communities.  This plan  provides an initial blueprint upon which we can move forward.

LHIN / Health Service Provider Governance Resource and Toolkit on Voluntary Integration Initiatives

This toolkit was developed through a joint initiative of five Local Health Integration Networks (LHINs), associations representing the health service providers funded through the LHINs, and the Ontario Ministry of Health and Long-Term Care. The  toolkit is now available on the Central LHIN website. 

LHIN Collaborative (LHINC)

This new collaborative structure (LHINC) encompasses all LHINs and all provincial health service provider associations.  This is a shift from the historical relationship of provincial associations with the Ministry of Health and Long-Term Care. In this new collaborative, LHINs and provincial associations would work together to promote integration and implementation of provincial strategies.
